If there are any changes to your student's medical information or insurance information during the year, please let the office know ASAP or come back here to complete a new form. Please be sure to read carefully.
Paper forms can also be picked from the Rise Kiosk in the lobby or from the Kidz area by the gym. Return completed forms to either the office, the children's minister, or the student minister.
